How to refer someone to Complete Choice

Send the referral form, or call 08 7009 4462. It helps to include the participant's plan dates and funding, the support they need, their current supports, and any clinical or behaviour risk we should know first. We respond with a named contact — a next step if we can help, and a reason in the same response if we can't.

Two ways to refer

  • The referral form — refer a participant. It’s built for coordinators, Plan Managers, allied health and hospital discharge teams.
  • The phone — 08 7009 4462, Monday to Friday, 9am to 5pm.

If a participant or their family wants to contact us themselves, the enquiry form is the right one for them.

What to include

A referral with these details lets us give you a real answer the first time:

  • the participant’s name and contact details
  • their plan dates and the relevant funding
  • the support they’re looking for
  • their current supports and providers
  • any clinical or behaviour risk we should know before we respond
  • your own details, so we can come straight back to you.

If you don’t have all of it, send what you have. We’d rather start the conversation than wait for a perfect form.

If the participant hasn’t given consent yet, say so on the form. We hold the referral and contact nobody until you confirm it’s been given.

What happens next

We respond with a named contact. If we can take the referral, you’ll get a next step. If we can’t, you’ll get the reason in the same response — so you can move on without waiting on us.

For complex clinical needs, send the detail. We’ll tell you honestly whether we can staff it safely, rather than accept a referral we’d later have to hand back.

Conflict of interest

We provide both Support Coordination and Supported Independent Living (SIL). That’s a conflict of interest, and we’d rather name it than have you find it. Where we provide both for the same person, we disclose it in writing at the start. The coordination team and the accommodation team are separate people. Whenever housing is discussed, we present alternatives to our own homes, and the participant can change either service without affecting the other.

Vacancy alerts go to coordinators before a listing goes public. Our current vacancies are always dated with the day availability was last confirmed, and our page for Support Coordinators shows capacity across every service.
