- Single-worker risk assessment. Where someone chooses to be supported by one worker alone rather than a team for daily personal activities, we complete a risk assessment before that support starts. It exists to keep people safe when there is no second person present.
- Supplementary Module assessment — scheduled at our mid-term audit, within 18 months of registration.
- Module 5A (Assistance with Supported Independent Living (SIL)) — assessed at the same audit.

SubscribeWhat we don't provide
Not registered for: Specialist Disability Accommodation, Plan Management, Specialist Behaviour Support, Early Childhood supports, Assistive Technology, Home Modifications. No day centre or group programs — our community participation is one-to-one. If that's what your participant needs, we'll say so on the first call and point you somewhere useful.
Every coordinator has lost a week to a provider who said yes to everything. We'd rather be the one you call first because we say no fast.
Questions people ask
Is Complete Choice a registered NDIS provider?
Yes. Complete Choice SA Pty Ltd is registered with the NDIS Quality and Safeguards Commission for 14 registration groups, approved until 9 December 2027. Three conditions apply to the registration, and they are published on the Commission's register. You can check the full record yourself by searching the NDIS Provider Register for Complete Choice.
How fast do you respond to referrals?
We respond within one business day, with a named contact. If we can take the referral, you get a proposed next step. If we can't, we tell you why in that same response, so you can approach another provider without waiting on us. If a referral is urgent, call our office as well as sending it.
Do you take complex clinical needs?
Yes, where we can staff the specific support safely. Complete Choice is registered for High Intensity Daily Personal Activities and for Community Nursing Care, so nursing can be part of a participant's support. Workers are trained and assessed for each person's specific support, as the NDIS Practice Standards require. Send the clinical detail and we'll tell you honestly whether we can take it.
Can you take a hospital discharge?
Yes. We accept referrals from hospital discharge teams, social work departments and Support Coordinators. Call our office with the clinical detail and we'll tell you what we have and when. We don't make turnaround promises we can't hold. You get a straight answer on the first call, so you can plan around a real date instead of a hopeful one.
How do you manage the conflict of interest between Support Coordination and SIL?
Complete Choice provides both Support Coordination and Supported Independent Living (SIL), and treats that as a real conflict of interest. Where we provide both to the same person, we disclose it in writing at the start. The coordination and accommodation teams are separate people. Whenever housing is discussed, we present alternatives to our own homes, and either service can change without affecting the other.
How do I hear about vacancies first?
Sign up for vacancy alerts. We email Support Coordinators and Plan Managers when a Supported Independent Living (SIL) vacancy opens, before the listing goes public, so you can put a participant forward early. If you need an answer sooner, call our office and we'll tell you what's available that day.
